You will need to fax the following information to (303) 484-3578.

Patient Information

Create a facesheet with the patient's full name, date of birth, name of facility and address and basic insurance information.

Insurance Information

Copy of the front and back of the patient’s insurance/ID card(s) and the facilities main contact information including contact’s first and last name.

Wound Information

Documentation about patients' wound(s) size, type, location(s) and pictures (if possible).
